Rules for Minors (under 18 years old)

We strongly encourage parents to attend with their children.

  • No one 13 or younger can attend the conference without a parent or official guardian.
  • A person over 13 and under 18 must be accompanied by a parent or official guardian, or be chaperoned by an adult (25 years or older).
  • Every parent of a child (under 18) attending the conference must agree to the Family Responsibility Acknowledgment form, recognizing that he/she will take responsibility for the oversight of the child. (If a parent is bringing more than one child, only one form per family needs to be acknowledged or completed.)
  • In the case of a teen being chaperoned, one of the parents sending the child and the adult assuming responsibility for the child must sign the Chaperone Form, acknowledging this chaperone arrangement.
  • A chaperone may not be responsible for more than 4 teens. (For example, two children with two of their friends, max., or one child with 3 friends, max.)

Chaperone Form and Medical Release (Download)

This form should be filled out by a parent/legal guardian who is sending his/her child(ren) under 18 years of age to the conference with a chaperone.

Chaperone Form (pages 1-2) >> Returned to the TSCM office

  • Signatures required from both the chaperone (page 2) and the parent/legal guardian (page 2).
  • Must be emailed (faithwalkers@tscmweb.org) or mailed to our office (PO Box 224; Worthington, OH 43085
  • Form must be received by: December 3, 2018.

Medical Release (page 3) >> Give to the Chaperone

  • Signed by parent/legal guardian
  • This form must be given to the chaperone and kept with him/her at all times during the conference.
  • Note: please do not mail to our office

Family Responsibility Acknowledgment (Download)

Every parent of a child (under 18) attending the conference is required to agree to the terms and conditions of the Family Responsibility Acknowledgment during the online registration process, recognizing that he/she will take responsibility for the oversight of the child. Or you may send the form to our office.

  • If mailing in a registration form for your family, this form must be included when returned to our office.
  • It must be completed and signed by the parent or legal guardian and emailed or mailed
  • Form must be received by: December 3, 2018.

CHILDCARE

No formal childcare will be provided at Faithwalkers. However, there will be a room available for nursing mothers, and another for parents to be with their small children during main sessions.
